Home Care

  

Home care aims to make it possible for people to remain at home rather than use residential, long-term, or institutional-based nursing care. Home Care providers deliver services in the client’s own home.

Home care services include help with daily tasks such as meal preparation, medication reminders, laundry, light housekeeping, errands, shopping, transportation, and companionship. Home care is often an integral component of the post-hospitalization recovery process, especially during the initial weeks after discharge when the patient still requires some level of regular physical assistance.

Board and Care Facility

A board and care home is a licensed 24-hour care property. These senior living homes offer room, board, 24-hour staffing, and assistance with things such as bathing, dressing, eating, toileting and medication management. Board and Care homes do not offer nursing or medical care, but they often provide greater assistance with personal care needs. Typically, a Board and Care only has a maximum of 6 patients. This puts them in a unique position to offer a more individualized and personalized approach to care.

Assisted Living Facility

An assisted living residence or assisted living facility (ALF) is a housing facility for people with disabilities or for adults who cannot or choose not to live independently. They can range from 6 beds to well over 200 beds.

Skilled Nursing Facility

Skilled Nursing Facilities are for seniors who require 24-hour monitoring and medical assistance. Patients typically suffer from severe illnesses including cardiac and respiratory diseases or are recovering from a serious injury such as a hip fracture, so they are unable to care for themselves. Skilled nursing facilities provide skilled medical attention-such as Physicians, RN’s, LVN’s and CNA’s, physical therapists and activity and dining servitions

Home Health

  

Home Health Services offers a wide range of services and programs that can be customized to your specific needs. Staff will develop a care plan in coordination with your physicians. The care plan ensures that all your health care needs are addressed. It reduces the risk of re-hospitalization.

Hospice

  

  

Hospice care is a type of care and philosophy of care that focuses on the palliation of a chronically or terminally ill patient’s pain and symptoms and attending to their emotional and spiritual needs.

Hospice is a program of health care and services designated to meet the special needs of patients and their loved ones in times of crisis and terminal illness. Hospice can dramatically impact the amount of pain and suffering experienced, providing quality time to live comfortably and fully, with privacy and dignity, in your own home or care facility. The services provided ensure that the dying individual will have the opportunity to prepare for death with dignity.

Care is delivered by a team of professionals who address the patient’s needs on medical, physical, emotional, and spiritual levels. The team consists of the patient’s attending physician, hospice medical director, registered nurse, therapist, home health aide, social worker, chaplain, bereavement counselor, and trained volunteers.

Eligibility requirements for Medicare & Medi-Cal benefits will be determined when a patient meets the following criteria:

The patient has a chronic/terminal diagnosis with a limited life expectancy if the condition takes the natural course.

Aggressive measures have ended, and the focus is now on palliation, pain control, and emotional/spiritual support.

The patient’s physician authorizes the hospice care.
